To start off this appeal has no effort in it and is the wrong format. The correct format is as follows:

I would suggest you refresh yourself with the grief report and the server's Rules / Agreements before you file another appeal. Also, I would suggest you put more effort into your appeal so that we know this won't happen again. Last time you file an appeal was in July. I was the one who handled both of your appeals and I hope you will appeal again, this time with more effort and the correct format.
