I had this on a minor scale while testing. My inv went from 46 to 45 before it went up to 62 because of a bigger stack being picked up.

EDIT: /Speed 6. Went to 63 items. Then to 64 with a second stack beginning to fill. Then the first went down to 63 without changing the second one. Then it went to 64 again without changing the second stack and down to 63 again where it remained until I picked up some more potatoes and reached 64+25.

By now I have a guess....could it be that the player is breaking crops too fast so the server doesn't have enough time to create a random number on how many items to drop? I assume this because @Imabankthing said that it works without a fortune tool, but not on non-fortunable crops. That would mean that it does not occur when the amount of items to drop is already predefined, for example with cocoa or sugar cane.

Just to add, I've seen this with nether wart myself but never tested it on other crops.
