    I've heard report of these happening on other instances of teleportation

    I believe this bug depends on the Portal you are entering.

    If a portal has some sort of block in front of it or does not have a block at floor level, it can kill you as you enter.

    This morning I was holding a mining spot for @bmanvman, and when I teleported him back he died on contact with the ground.
