I have had this problem before, it happens a lot to people that I know.
To fix this, either press your right shift key or block a couple times in Minecraft.

That appears to be lag related, not hacking. If you feel it is hacking, however, we need a video. Sadly, screenshots generally do not show enough to tell hacking.
