Minecraft Username is where your Minecraft Username goes.
Reason is the Reason for why you were banned. [You can check this by logging back into the server to see what the reason for your ban was.]
Counter-Reason is your arguement against why you believed you were unjustly banned. Please do not fabricate stories as we can check through our chat logs.
Evidence is any screenshots/videos you may have to justify your Counter-Reason. This is optional but greatly enhances your Counter-Reason.
