All you need to do is create a thread using the format I have provided below. Fill it out completely and we will take care of the locks for you. Make sure to do /seen <username> to see how long the user has been offline. If it is not longer than 7 days then you will need to notify the user via the forums or in-game mail asking them to remove the locks within the next 3 days. Take a screenshot of this for proof of notification and include this in your request if it is needed.
