That error code doesn't make sense. Were you doing anything else?

 

That scenario also doesn't make sense. Could you replicate it with the a video?

 

I think Bandicam is a free program for recording. If you catch me online, I might try replicating it on camera for you.

 

Alright. After some testing, I've found out that you fall off horses if you have fly disabled. I tried to replicate this but was unsuccessful until I turned fly off.

When I rode on a horse with fly disabled, I fell off every few seconds. I even got kicked by anti-cheat a few times with code (004). I wasn't doing anything else at the time except riding a horse, and I got kicked when I landed on ice.

 

I don't have the /fly feature and was also kicked with error 004. I made an imgur page for the screenshot:

 

I've found the specific issue and made @JamieSinn aware of it.

It's not the ice it's the carpet happened to me too.

I think you had carpet too didn't you? Also I get kicked for carpet on top of ice and plain ice.
